I have created my own custom Tee Pipe Fitting. When I try to convert an elbow on a pipe run to this tee (using the small + symbols) it throws an error. I'm sure I have done something simple incorrectly...

Can you use the fitting in the project normally, meaning drawing a pipe tee rather that using the "+" icon? I think the problem here lies in the Family's geometry; in the Family Editor, I tried changing the value of "d" and it returned a whole slew of constraint issues which results in deleted extrusions. This is likely causing the error in the Project, since the Connectors' hosts are being deleted.
Still not really understood this one but the fitting works now...
I redid the 'body' that was defined by parameter 'd' by using sweeps instead of extrusions with no joy.
I then removed the nested family (that defined the socket) and made the sockets as blends. This then worked.....odd since I have used nested families without any problems on other fittings....
Thanks CADastrophe for pointing out that there was a modelling error somewhere causing the problems.....
