Specific Duct Fittings in Revit

So Revit is gret at making automatic duct fittings. Is it possible to get it to make something like this:

Would I have to make a family for it? or is there somewhere that oyu can download common fittings such as this?

If you're talking about a specific offset transition fitting, the OOTB Family covers that, as will most manufactuer's Families. Revit will automatically create such a fitting if two Ducts have the correct justification and are aligned on one side, instead of on center. Create both Ducts, select one segment, and use the grip to drag and connect it to the other, and the offset transition should be automatically created. See attached image.
