Menu Tab

Our menu tab is cycling through the menu tab by itself randomly (from 'minimize to tabs' to 'minimize to panel titles,' to 'minimize to panel buttons').  Sometimes it happens after every 5 or 6 commands.  Is it possible there is a shortcut that does this that we are pressing and don't know about?  We cannot find this item item in the 'keyboard shortcuts.'

In the past we have seen odd behavior when workstations have multiple monitors in place, or a large monitor set to a high resolution. These issues are magnified if Revit is being utilized in the secondary monitor, as Revit attempts to use the primary monitor by default. We have also seen these issues if the Project Browser or Properties Palette undocked from the main Revit window.

If you have not already, I would verify that the monitor DPI is set to 100%, Revit is on the main monitor, and the Properties Palette and Project Browser are docked.
I would also make sure all available updates are installed on teh machine. You can check by going to Help > About. There are 2 updates for 2012, and 3 for 2013.
