Annotation Balloon

I am trying to create an 'Annotation Balloon' and have no idea how to start.  By 'Annotation Balloon' I mean something that  looks like a 'Callout'  with a note attached to it but does not create another view but simply surrounds some graphics in a view that have an association.  Whilst this can easily be achieved by drawing some detail lines and having a text leader pointing at it (although it is impossible to draw 'detail lines' in a 3D view, where I would expect to use this device mostly), I would like to automate this.  In actuality what I want is a leader head in the style of a 'callout' balloon with a tag being the title text.

I attach a part sheet view where I have approximated what I am after.

I created the "bubble" as a Detail Item Family with adjustable size via dimensional constraints/parameters, then created a Detail Item Tag, and used the "Comments" field to type the note.

Do you have a leader line associated with it?

The Leader would be part of the Detail Items Tag, just like any other Tag Family.
