Listing blocks from standalone app

Just to make sure I'm getting this right, if I have a standalone application that I'm writing in VB.net, I will have to continue on using the traditional way like:
For each layout in layouts
For each entity in layout.block
etc...
is that right?

And only if I have a dll loaded via AutoCad then I can use the database and transaction process?
