The start and end tangents are vectors that can go any direction. Setting start tangent to a vector from the first to second point will flatten out the start of the curve.
To get a better idea, run the spline command in Autocad and after drawing a few points hit the space bar to specify a start tangent. Drag the cursor around and watch what it does to the curves.
