Vault refusing to Insert a file from Vault

Publisher 2013, SP2, Windows 7.

I am trying to insert an IAM file from vault.  I get the following warning:

I am getting this for every file I try.

I can insert a file that is not in vault.

I would really appreciate a quick answer.
