sheet metal bend problem

I know this is elementary, but I'm having problems forming this part up to get a flat for the sheet lasers.

What would be the best way to start this part?

I made a similar part recently. This is how I did it. There may be better ways.

If you attach your ipt attempt here we could figure out what you did wrong.

Not sure if there's an easier way or not, but that works perfectly.
