Are you using the same project (.ipj) when you edit your template and when you start a new file?  If so, can you post a screen shot of that project open in the Project Editor, with both the Appearance Libraries and Material Libraries expanded?

I have the same problem in 2012.  I have purged and updated and saved to the styles library many times, and no matter what I do, this message continues to appear whenever I use one of my templates.  I can't make it go away.  Many of our users have this issue.

 

This only started happening about 6 months ago or so.  Maybe related to an update?  I don't know.

Well my templates are stored in a different folder.  They are not in that project location.

Right, but I'm trying to find out if you are using the same M & A libraries when you are editing the templates as you are when you start a new part and get this

It appears that there was a subfolder with copies of the templates I was using and I was opening them by mistake.  The templates I'm actually using open up fine.  The problem has been solved... er well, it's been solved for a while now, I just did not notice because I was opening the wrong templates.
