molding two features together, smooth transition

Trying to figure out how to mold a pipe and flange together wit a filled in transiton as if they were one continuous piece.

The pipe edge is flush with the flat square fange.  The combine option does not allow me to conduct operation.

Any assistance would be greatly appreciated.  See attachment.
