Making tube between two circles

I've been trying to make tube between these circles but I can't figure it out how to make it smooth. I tried sweep too but that didn't work too well. Here's picture of my try without sweep: http://i.imgur.com/xvGyZ.jpg

You see that it leaves corners on the back. I would like to have it so it's smooth and has same angle. How should I do it?
