AutoCAD Civil 3D 2009 has two startup shortcuts. If you need the AutoCAD Civil 3D user interface make sure you are using the "AutoCAD Civil 3D 2009" shortcut. Both shortcuts launch the same acad.exe but differ in their command line options. A description of the two startup shortcuts follows:
The "AutoCAD Civil 3D 2009" shortcut has command line options that load an AutoCAD Profile called <<Unnamed Profile>> and the AecBase.dbx file for Civil 3D. The Target and these command line options for this shortcut are: "C:\Program Files\AutoCAD Civil 3D 2009\acad.exe" /ld "C:\Program Files\AutoCAD Civil 3D 2009\AecBase.dbx" /p "<<Unnamed Profile>>".
The "AutoCAD Civil 3D 2009 as AutoCAD" shortcut launches full AutoCAD 2009 without the AutoCAD Civil 3D 2009 code. Its only command line option is to load an AutoCAD Profile named "AutoCAD". The target for this shortcut is: "C:\Program Files\AutoCAD Civil 3D 2009\acad.exe" /p "AutoCAD".
Try launching from the Start menu > All Programs > Autodesk > AutoCAD Civil 3D 2009 > AutoCAD Civil 3D 2009. These startup shortcuts are less likely to have been modified and shortcuts that may not be on the desktop should be listed here.
How you start AutoCAD Civil 3D or how you may have modified these targets may cause AutoCAD Civil 3D to load with the AutoCAD Profile of the last use. You may need to go into Options (you can always get here by typing "options" at the command line), choose the Profiles tab, select the <<Unnamed Profile>>, and click the Set Current button. Click OK to exit Options. As long as the <<Unnamed Profile>> has not been changed the AutoCAD Civil 3D user interface should load.
You may need to recreate the "AutoCAD Civil 3D 2009" shortcut. Hopefully this gives you the required information to solve the issue you are having and if necessary recreate this shortcut.

The <<unnamed profile>> must not be configured for Civil 3D anymore. At this point it is probably best to delete the user files and Registry entry then let the Installer rebuild them. You will find the instructions for this here:

We found that you can type "menu" in the command line then browse to the "civil.cui". Once that loads all seems to be good. It was a lot easier than deleting folders and editing the registry.
