Stretch Both Directions not parallel

See attached DWG.

Two offset arrows.

This works as long as it is not rotated, but distorts if rotated and then stretched.

I'm using ACAD2006.

You have two problems here, the distance parameter doesn't need to be chained because it works independently, and the two stretch actions shouldn't be included in the rotate selection set.

Not sure what you are referring to when you say Chained.

But nothing I do helps the problem of stretching after rotation.

I agree on the chained action, but not about excluding the Stretch actions from the Rotation selection set

You are indeed correct I took out the actions first from the selection set, and then spotted the chained parameter, but it is unnecessary to have the stretch actions included. I tend to keep them off to the side of my blocks with notes about what each action does.
