Get up to speed - latest raster to vector converions tech?

Can someone direct me to the latest info on Scanning in paper drawings
both for archival and for e-file creation to be used as a editable
Autocad drawing.

Software and Hardware...
Best format for scanned images...
Raster to Vector conversion software...
Going rate for such services...

Are their scanners available that will put out vector format in one
step?

We find it works well to just import the scanned image into acad with the
image command. When changes are needed, we use imageclip to cut out the
changed areas (or erase in photoshop) & draw over it in acad. Vector
conversion can be troublesome & expensive, depending on the type of
drawing.
