Error in Raster Design 2004 w/ ADT4

I'm getting an error when I try to run Raster Design 2004 in ADT4.0 or
Acad2004. Basically when I type the "Image" command, I get a message
stating: "ISM Context Menus did not load". When I manually load the
AECO.mnu (Raster design menu), none of the arx functions for raster design
works and I then get an Exception 1F error loading the raster design arx
file such as AECIREMU140.arx etc..

Any ideas? I've uninstalled/re-installed ADT40, ACAD2004, and Rasterdesign
2004 and still get this problem.
