Missing Valve Operator (Custom Block)

I have recently started using custom blocks to add electric actuators to various valves in a project I'm working on.  For awhile it was working fine however now the valve blocks are no longer displaying the actuator.  It seems as soon as I delete the original block that these valves were using to show the actuator it will remove them from the valves if I close and reopen the drawing.  Keep in mind I have not purged the block from the drawing just deleted the physical block.  I would assume that I shouldn't have to keep a block in the display just so that the other valves work right.  If I am wrong I recommend a fix for this in the next release.

Just to clarify, are your actuator blocks in P&ID or Plant? Are you using 2013? Do you have any before/after screen shots you can share?

The problem I'm having is in Plant 3D.  Attached is a picture that shows one valve without it and one with the actuator.  I have since started to reselect the blocks so that it shows again.  If you notice I highlighted what properties says when you select the one that does not show the actuator.

The Block In Plant Not P&ID, how it will be in p&id And my screen shot saw the generated isometric?

There is no P&ID for this job I am simply modelling piping for coordination.  Well there is a P&ID but it wasn't created with Autodesk P&ID, just regular AutoCAD.  If I were to do it in AutoCAD P&ID it would just be a control valve.  What I am trying to do is just attach a 3d block of the actuator (in this case a rotork actuator) via the valve operator dialogue box.  See attached image for clarification of what I'm clicking on in a 1, 2, 3 step sequence.
