Equipment separating into various pieces

Hi,

I've created a piece of equipment with various simple 3d objects and another piece of equipment attached to it.  When I leave the file and come back into it some of the objects move and rotate away from the equipment, but they are still attached to the equipment.  It hasn't done this from the beginning, but just started.  It occurred once before, I detached all pieces, moved them back in place, re-attached them, and it did it again with different pieces.

What could be causing this?

I'm having this issue now as well.  I'll create a vertical vessel, create a skirt for the vessel with extruded circles and such, then attach to the equipment.  After awhile, say later in the day or the next, I'll notice the skirt in a different location.

Any clues?

I'm running 2014 with Extension 1.
