I have create new project(custom made) and when I try to convert AutoCAD 3D block to Plant 3D I got this message. What is the problem?

Is the item you are trying to convert a Block? You might try to explode it and then try to convert it to see if that helps.

My thought also.  Start out with a named AutoCAD block.

In case you need it, here’s a brief outline of steps for creating a Plant 3D custom part from an AutoCAD block:

  1. Create part shape with AutoCAD 3D modeling. (In Plant 3D, you may prefer using 3D Basics or 3D Modeling Workspace)
  2. Using the BLOCK command, save as an AutoCAD block. Name it, select insertion point, select convert to block, select OK
  3. In 3D Piping Workspace, Home tab, Part Insertion panel, select Custom Part button .
  4. Select AutoCAD block then Create AutoCAD block based part (PLANTPARTCONVERT command).
  5. Select the AutoCAD block
  6. Add ports (also can remove, delete, exit)  Use osnap, select where pipe will meet object. Specify direction pipe coming from (have ortho ON).  Note that Actuators don't require ports.
  7. Accept and Add another or eXit.
  8. Now select ‘Choose block from model’ *  [In custom part dialog, when you say select block, it's expecting a plantpartconvert-ed block)
  9. Select block you just added the ports to.
  10. Select Part Type choice
  11. Select Insert in Model
  12. Test new part by pulling pipe from the ports.

*Note: If you get “No ports information for block” message, this can be fixed by installing Service Pack 1 for Plant 3D 2014: AutoCAD Plant 3D 2014 Service Pack 1

Here's a short video demonstrating these steps.
