Product or add-on?

We are using vanilla AutoCAD with some customization to draw P&IDs. It is pretty good, fast and able to extract majority of information. So why Autodesk does not think that they should make the P&ID module should be an add-on feature for the existing AutoCAD customers at some extra cost?

It essentially is an Add-On - all of the functionality of AutoCAD is included.

Does the add-on mean that, if I am already having AutoCAD 2007, I can get only this P&ID module at an extra price? I mean, I don't want to buy another AutoCAD for this P&ID module. Is it possible?

Yes, ask your sales person about a "cross-grade".
