Fatal Error: Unhandled Access Violation

I have been getting the following message on MEP 2012 (running on Windows 7):

Fatal Error: Unhandled Access Violation Reading 0x2d1a0680 Exception at e75800d7h

This usually happens when closing AutoCAD using the red X in the top right hand corner with multiple files open.  It will close everything fine, but then throw an error as it finishes closing.  I can not get this to happen regularly, and it happens maybe once every-other week with daily use.  I have not lost any drawings or revisions made to them when this error happens.

Any help is appreciated.

AutoCAD has also been very glitchy lately; the pickbox on the crosshairs switches between square and rectangular, and switches between drawings at random.  Also, I used the 'match properties' tool and now the paint brush stays with the crosshairs eventhough the command is finished.

Not sure if the two are related??
