Missing Text Formatting Toolbar

When I edit multiline text with either ddedit or mtedit commands, the text formatting toolbar does not appear. The ruler and the text appear but not the formatting bar. This is within LT2009. Can anyone help?

This has always happened, just started, or limited to the one example before
posting, did you recently change from dual-screens, were running an external
screen and now you're not... please elaborate.
