Dimension arrowheads disappear

Occasionally arrowheads will disappear and they seem to be on a different layer, but I can't figure out how to make them appear on the current layer.

You have an errant block definition: easiest fix is to start a
new blank AutoCAD session, set up your dimensioning using DDIM, draw a dim or
two on Layer 0 (zero), then INSERT your problem file into this one, EXPLODE, and
save as old name.

Or, redefine the block you are using to be on Layer 0
(zero).
