Changing Menus

I have added a custom menu and a sub menu to the main menu bar but I cannot change the name of a submenu nor any commands under it.

Hopefully the attached picture will clarify things. I had to drag a command from the command list onto my custom menu and change the manco because I could not find a way of adding one from scratch - have I gone about this the wrong way?

Also, how do you know which 'menu customisation' file you are editing? I have supposedly setup the system to work from a network so when I change the file I would have expected everyone to have the same menus however they don't. I do not know the file name to look for to see where it is located and how it fits in with the support paths etc.
