According to THIS site.Cartogramma is included in the 2015 Productivity Pack. I don't use 2015 yet so I don't have any direct knowledge. I'd suggest asking your reseller.
As far as not needing Subscription. You don't have to install the product but if you skip a subscription upgrade you lose any rights to upgrade in the future. Again. Talk to you reseller.

In typical Autodesk fashion, they have used similar names for different things....In October they posted a number of global and regional Productivity Tools for C3D2015, including those listed on the site you linked to. However, the readme says that the Cartogramma tool is only found in the Russian specific tool pack. Unfortunately, I don't see that particular package in the downloads section. The UK & German packs are there, but no Russian pack. Then in December they released the Civil3d 2015 Productivity Pack 1 which contains just 3 new tools: Open Infraworks Model, new COGO Editor, and Create Autocad 3d solids from some C3D objects.
So I don't see any way to get the Cartogramma utility for 2015 at this time.
Thanks for clearing that up Jeff. It's often difficult too because Autodesk sometimes only mentions NEW additions to a tool and doesn't recap what else might be there.

It is in the Russian Pack but it is in a mixture of Cyrillic and standard Western fonts so this could be interesting....;)
