I change my surface style to slope banding (3 ranges) and make a legend table.
All of the three ranges in the table are red and my surface ranges are blue, yellow, & red.

Yes I did. It only shows the color of the steepest slope.

For some reason it's not letting me attach a dwg file.

Perhaps all of your slopes are in the red upper range of your analysis and there are no flatter blue or yellow areas.
For my table style for slope, on the data properties tab, the structure below shows that my "color" is set to <{surface range color}> is yours set to that?
I was able to figure out that you figured out how to attach your cad file! So i played with it. I redid your surface analysis. I first selected the slope type. I then changed the ranges to 4 and hit the arrow. Right away i changed the ranges back to 3 and hit the down arrow. The colors and ranges changed to some sort of default. I adjusted the ranges to match 0-.5, .5-2, 2+. The surface you had and the label box showed the expected colors for both (all different).
Default colors of 26, 66, and 96 seem to work as expected.
So, I clicked on that and went back to analysis and changed each color to red green and yellow. That's when the whole surface turned red and the legend colors all turned green. That's also when I will leave it up to you to.
BTW - the only weird thing I noticed was that the original look at your surface anyalysis the minimum slope of the first range did not start at 0, but at 0.0028%.

anyway, I couldn't fix your style, but i could drag one of my working styles into yours and got that to update and look correct. Please check it out. Compare the two if you like or just use mine from now on. I didn't bother to do a full comparison. i did notice the layers we use are 0 instead of all the c-anno...

I repeated your workflow several times and it appears any color combination works except colors 1-6. When I used 7,8,9 or several other variations throughout the color chart, the table updated accordingly.

Very interesting color limitation there. please see my attached file if you would like to use all colors, including 1-6. I am not sure about the differences in style, but maybe you'll find it and post back. Have a great day.
The different setting we had that made a difference was the "sort data" check box. (see attached)
When I unchecked it in my style, the table updated correctly.
go figure.
