Multiple Layers Wtih Same Name/Identical Layers

I can't believe this is possible but here it is. [Please see attached screen shot.]

A beer for the first person to tell me what was done to create these, how to fix it, or to offer me a new job.

Have you tried Purge?

Layon
Regen
wblock out the entire dwg.

if that fails...

Explorer>Directory>select file, press "delete"

See image below. Next time don't get your layer manager wet! Sorry I can't
help fix it. BTW, some of the layers are not identical, but have different
colors.

Update:

I first dettached all xrefs, then purged and then ran an audit.

The layers have disappeared, but before running purge and audit I tried to access the layer manager and received the error message attached. I've also attached the output from the commands I ran.

This appears to fix the layer problem, however I would still like to know if anyone has seen this before or has any insight as to how it was caused.
