Label sizes in the viewport

I am having trouble getting my spot elevation, slope and alignment labels to appear the size specified in the style through the viewport. Sometimes they are ok and sometimes they are not. Sometimes they disappear. I am using Civil 3d 2009 and my units are m. Any ideas...

My plot scale is set to mm and my annotation scale is 1:500 which matches my viewport scale (XP factor = 2). When I create a new viewport the labels appear. When I change my UCS and do the plan command, I lose my scale. I regen, and set my scale up again only to lose my labels. Sometimes only my surface labels disappear and sometimes the alignment labels and the surface labels disappear. It makes no sense to me.
I am teaching a Municipal Design course and don't like telling the students to use trial and error.
HELP!!!

You probably will want to look into the label style's orientation reference. If you are using UCS, you may want to use View as the reference.
