You don't say which version you're using but you could use either LaneOutsideSuper or LaneSuperelevationAOR then BasicCurb followed by UrbanSidewalk. For the final piece I suggest LinkOffsetOnSurface. That will allow you to tie the section to the existing surface at the 15m boundary without a specified slope.
