Civil 3D 2009 Unhandled Access Violation?

Civil 3D 2009 Unhandled Access Violation?

I'm running Civil 3D 2009 and whenever I shutdown, I get the following error:

FATAL ERROR: Unhandled Access Violation Reading 0x0058 Exception at 6367cb7bh

Any ideas? I gets frustrating because when I start Civil 3D the next time I get a message that it unexpectedly crashed and I can go through the recovery process.

Civil 3d 2011 or 2012
Quad Core Intel
Windows 7 x64
8GB RAM

Have you installed Update 1 for Civil 3D 2009?

If you have and continue to see this issue, please submit a CER report
so we can investigate further.

I have update version 2. I guess that means I do a CER submission. How does one do that?

No pop-up occurs when I get the Fatal Error. How does one then submit the information on a CER?

Unfortunately the CER window isn't popping up so that data can't be
sent. I'd suggest a repair of your installation first. It would be
best to open a support case through your reseller or directly with
Autodesk subscription support to debug this. Good luck,
