Undesired Print Rotation

Haven't had this problem going back to r13 (not 2013... 20 years ago).  I've tried new drivers, I've tried playing w/ PLOTROTMODE, I've tried creating a custom page size, I've investigated driver print rotation settings.  Everything looks proper in the plot dialog mini-preview, but the full print preview (and print) are screwed up (see below).  Thoughts?  This is in ACAD 2015, Windows 7, PC is an HP Z420.

Expand the Plot Dialog box, there are more options.

Certainly.  Which part would you like to see expanded?  Properties?

The Arrow lower right corner next to HELP button.

Gotcha... mostly work in Inventor these days.  So I expand that and I see "Drawing Orientation" is in Landscape... AH HA!  Except... no... still no... see below.
