SURFPATCH

I am teaching myself AutoCad 2015 using AutoCAD and Its Application by shumaker and Madsen.  I am attaching two files:  EX10_4.dwg and instructions file ex10_4.pdf downloaded from http://www.g-wlearning.com/cad/  "Advanced" Chapter 10.

I can not make the command SURFPATCH perform per the attached instructions.  I accidentally got one patch to work using Continutiy G1 and Bulge setting .5) but could not repeat the operation. Nor could I get any of the other patch exercises to work in this drawing.  What am I doing wrong?
