Instructor Code / Certificate

Good Morning, 

 

Can anyone tell me how I can get my Certified Instructor Code from AutoDesk.  I became a Certified Instructor years ago and I have forgotten what my code is (getting old).  

 

Also, is there a way to get a copy of the Certification ?
