Annotative Door Tag

I am currently having an issue with an annototive door tag and I have tried everything discuss on the forum to fix it with no luck. The bublle of the annotative tag dissapears when put to 3/32" scale. This tag currently have 2 scales (1) @ 1/4" amd (2) 3/32". At 1/4" the block works perfect, I also cannot delete the annotative scale at 3/32" for some reason. I have tried audit, purge and cleaning the file but nothing seems to help. Please revie file and somebody let me know what the issue can be?

I am currently using Autocad 2011.

Hi, 

Delete the existing one. insert another one then add scale 3/32" to the new one.

i did it as mentioned above and every thing is OK !
