Vault Professional 2013 - Visualization

We are experiencing the issue with preview of Vault, Whenever Inventor creates a visualization file, it appears to be alright but the moment we try to update a visualization file by clicking update locally in preview tab of Vault the file created is mostly not right, it appears only on revsions, seems like Vault when creating visualization file got confused with different versions of part file edited.

Explanation with Example - Lets take an example of one drawing file called A.dwg which is drived by assembly B.iam , B include 3 parts C.ipt,D.ipt and E.ipt , Till the revsion A everything seems right but let us suppose we go to rev. B of drawing A.dwg and we change part D.ipt only, now visualization created by inventor is still fine but when we updated locally on Vault,  Preview clashes  with different versions of part D.ipt  and preview of the drawing file A.dwg  shows missing or unreferenced dimesions with no changes showing for part D.ipt

If anyone have experiened it before, your feedback is greatly appreciated.

Please tell me, do you have enforce consistent working folder all clients, set? Please clear out the users working folder then run the update.  Do you still receive the same results?  Also, how many different ipj files are stored in your Vault?

Thanks Ron, for the reply,
We have a enforce consistence working folder and have only one project file, The task of updating the document is normally done by Document controller after the drawing is approved and They don't have local copies as they just synchronize the document and Update
But i will still test it again and come back soon
