I have set the Vault option to Restrict Item Lifecycle Changes to Change Orders. I am currently the only Administrator in the Vault, but my users are still able to manually change Item Lifecycles.
Any thoughts on this?

When I tried to reproduce your issue by Right click on an Item and choose "Change State", I get the following dialog:
1. Could you describe the steps your users use to change the state?
2. Are they always able to manually change the state or does it only happen under certain circumstances?

I found the answer. Previous admin had placed all users into a group, and gave the group admin privelages. We were not using Items or ECO's at the time. Once I reset the roles on the group the behavior was as expected.
