I am using Vault 2012, I have an ECO that is basically stuck. I have the Item in the "Work in Progress" state, allowing me to make changes to the files. After I make the changes, the file does not update in the ECO, I can usually get around this by Updating the item from within the ECO. For this ECO, it tells me that there are no newer files available. However, when I get fed up with it, and decide to get out of the ECO and try again, I can't change the state of the item to "In Review" because "Operation cannot be performed on an old version of the file that is associated with the item."
With ECOs in general, if I have the ECO open and Vault freezes or crashes, the ECO remains locked for about 30 minutes. When I try to open it for edit again, I can't because it is locked by me.
Does anyone have any idea of what workflow missteps may commonly cause this? I have two clients that encounter this frequently, but I can't figure out what to tell them not to do.
I have one user to whom this has happened a few times, but I cannot nail down the workflow that is causing it... I think I'd have to be standing right over his shoulder watching him when it happens. As it is, by the time I get the call the damage is done.

This just happened to me! Bad enough when a user cannot edit his or her ECO, but what is the Admin supposed to do when the ECO is locked?!?! There has to be a fix for this! I'm dead in the water until it decides to unlock.
