Adding sizes to the XML file should work fine. You'll need to close and
Re-start Revit Systems for the new sizes to appear in the list.
For duct, you can also simply type a new size in the size combo box if you
are doing a "one off" size and don't always want it to be available.

I just got a reply from John Henry in regards to a question I had about Pipe
Sizes. His reply may help you with your question. I've modified his reply
to work with ductwork instead of piping.
Go into Settings/Project Units/HVAC, the default Duct units are set to whole
inches, just change to nearest 1/2" or whatever you
may need.
