Piping visibility problems based on detail level

Im having two problems with my sanitary piping which seems to be dependant on the detail level of the views.  If I keep things at the fine detail level it all shows up great.  But if I lower it to med or course I get missaligned piping at fittings and piping that is not visible as shown in the two attached pics.

 

Any idea why this is happeneing?

I take it that these are not out-of-the-box fittings? The problems is probably that the Model Line that represents the fitting in single-line within the family is not correctly controlled or constrained.
