Model section of rod with cylindrical end

I need to model up a length of rod with a cylindrical end with a hole in it. The cylindrical end needs to be flattened slightly where the rod meets it. Can anyone provide a step by step guide on how to do this ? Please see attached picture giving an idea of what I am looking for. Thanks.

Here is one option of getting it done. Please see the attached. The part had been done years ago - so don't think of it as being a perfect one.Smiley Happy
