Cannot Activate AutoCAD LT 2014

I cannot seem to automatically activate my AutoCAD 2014 and when I choose to perform the offline activation, the form for my request code information never pops up.  Is there another way that I can get to the request form so that I can perform the offline activation?

I cannot automatically activate for some reason.  I am trying to manually activate, but never get the form as mentioned in step 5 of this link
