Autodesk Vault

Has anyone seen this before?  In 2012, I did a search in the web client in items and found the desired item.  When I go to that item's attachments, I see two listings for the same IDW.  Both links open the same file.  When I look at this in the full client, I see the single link to that drawing. When I search files, there is only one.  Is this a defect?

I saw it also,only once it happened to me. 

In the Vault client you see only one drawing but the web shows two. 

I closed it and reopened since thne it has not happened to me.
