Mesh Refinement - Odd Mesh Contact Points

I've included the picture of the issue I've been having; basically I mesh the model, set refinement points and after doing so, the mesh shows up, in some areas, as shown in the picture.

Any reason that this happens?  Having to go back and redraw the mesh by hand and solid mesh is overly time consuming.

Thanks in advance for any and all help.

Having seen this problem before, yes, these are real elements.

Nonetheless, here is a picture of the area that has been shrunk.

No help or suggestions from any Autodesk employees?

I'm assuming this is happening due to the fact that midside nodes are turned on.  Rather than the mesh connecting nodes at the edge of the part, it appears that they skip the first row of nodes on the first elements and proceed to connect at the midside node locations instead,
