I am noticing the same issue like you. When creating annotaions with additional geometry to Plant 3D attributes it causes this problem. Only thing that worked for me is setting the basepoint of the Plant 3D attribute to "middle-center" and 0,0,0 WITHOUT any additional geometry.
If i add geometry or move the attribute basepoint from 0,0,0 to another location it´s changing the location everytime you enter the block editor of ANY annotaion block into your setup even without making changes to them.
It might be someone out there who got the solutions for this problem/bug
