Microsoft Access customized form when drawing a new element

Hi,

I want to have a customized Microsoft Access form being prompted every time i draw a new element on a FDO layer. At the moment the complete Auto Cad Map data table is prompted. I want to use my customized forms where i can insert images, documents, ive got my comboboxes, etc...

The idea is to link it by an ID if its posible.

Whats the best solution for this?
