What kind of issue? In general, we call any issue where objects are over a mile from the origin a "Large Coordinare Issue," and yes there are limitations. Once you get about a mile from the origin, linework becomes less accurate.

This is not a concern unique to AutoCAD MEP... it is a general problem of any digital application dealing with huge numbers.
The way you can deal with 'real world coordinates' in AutoCAD is to utilize the GEOGRAPHICLOCATION features.
