MEP 2012 Pipe Fitting issues?

I'm having issues with 2012 MEP pipe fittings. Some insert the wrong size despite my specifically asking for the correct size, some also insert incorrectly... transitions for example will occasionally insert not only the wrong size but with the larger end on the smaller pipe and the smaller end on the larger pipe. i'm perplexed... been using MEP since 2008 and I'm no expert (like many I've seen in this forum) but it's not often i'm seeing wacky things like this that I can't figure out on my own.

Eric,

 

I meant to get back to you sooner, but work has been very busy.

 

one example would be the 3/4" copper T... when we try to insert one it always inserts as a 3" T.

 

I know there are others as well and I'll try to list them as I run into them.

 

Has anyone else had this problem?

I have had a similiar issue with reducers, being wrong size and inserting backwards.  I have only seen it do this on one drawing and only certain sizes/types of reducers, I did try and reload the catalog and it didn't change anything.

Your jpg captures exactly the other situation I've run into.

 

Copper pipe reducers inserrting both incorrectly sized and backwards.

I haven't been able to readily reproduce this... can you provide a drawing along with specific steps to reproduce?
