The keynotes feature is an AutoCAD Architecture feature (like walls) that MEP inherits.  The default keynotes are Architectural.  So not many MEP users, use this feature.  I am not aware of a way to keep it open.  You should re-post this question in the AutoCAD Architecture forum where you will have users with more experience with this feature.  They may have some tricks that I am not aware of.  And if it cannot be done, consider posting a suggestion at the feedback site here.
