The Architect at my work had the same problem with this today and i figured it out just by fluke
type in ANNOALLVISIBLE. it should auto fill set it from 0 to 1 and you should be good to go. btw you need to be in paper space to do this and you need to do it to every layout.

1) Go to paper space
2) Double click inside the viewport
3) Type "annoallvisible" and modify the value as 1(one) from 0 (zero)

I had the same problem recently. A block i created from scratch for a window did not show up in paper space. All viewport layers were set to on and not frozen. I am using AutoCAD 2015. It turns out when you create a block, one of the property settings is "ANNOTATIVE" (YES/NO). The default settign is YES. using teh block editor, i changed this setting to NO, and it fixed teh problem. It has something to do with the annotative scale settings and the block not showing properly in your paper space viewport when this setting is turned on.
Did not work for me. I am still having the problem. I have tried inserting the block in both model and paper space. It is visible on the screen bit not on plot or plot preview. I have tried all of the tricks mention and before you say it, no...my layer is nor off or frozen and is set for plot on. Please help anyone.
