Have you tried the alignment reports from the tool box tab?

    Thank you

Go to Add Alignment Labels... Select the following Label settings.

    Choose Feature: Alignment

    Label Type: Point of Intersection

Point of intersection label style: Create a Style that labels exactly what you've listed.

Under the Layout tab, under contents add the criteria that you're looking for... See picture for example of the Civil 3D out of the box style. I usually start by taking one of stock styles and modify specifics. Good Luck.
