REALLY strange layer name/reference loop...

"12292-Base" is the drawing that I am in. Somehow, all of the layers in this drawing took on the infinite prefix shown in the photo. What in the blue hell happened, and how do I get rid of it?

Got all the latest hot fixes and service packs for your version?
Mainly the scalelist hot fix.

Did you run the scale clean up utility on all the xrefs?

Yes, there is one nested Xreference of a PDF that was used to trace in some linework.

Well I ended up just deleting the prefix to each layer, one by one. And there were a lot of layers, so that's why it took me a month to follow up. The scale cleanup and nested Xrefs investigation didn't fix the issue, but so far it hasn't happened again either, so maybe it was a preventative measure.
